- Parents of incoming 7th graders – Please don’t forget to submit an updated immunization record showing that your student has received both TDaP and meningitis vaccines. Both are required by the state for entry into 7th grade. The school nurse has contacted you via email if your student’s vaccine record is needed. Students may be excluded from school if the immunizations have not been received by the school nurse if not received by the first day of school.
- Parents/Guardians of incoming 7th graders – Don’t forget to send in your dental examination forms!
- Just a reminder that a doctor’s order is required for any medication to be administered during the school day (except those listed on the green emergency card), including over-the-counter medicines. Please contact the Health Suite at extension 3515 if you have any questions.
